According to Article 13(1) and (2) of the Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (OJ EU L119/1, 04.05.2016), hereinafter referred to as GDPR, we inform you that:

The administrator of personal data is ADENT Dental Practice Rafał Kleszczewski, Aleja Niepodległości 210/1, 00-608 Warsaw.

You can contact the Data Administrator by mail at: ADENT Dental Practice Rafał Kleszczewski, Aleja Niepodległości 210/1, 00-608 Warsaw.

Your personal data obtained by the Administrator will be processed in order to respond to the email sent via the contact form, that is, to take action at the request of the data subject before entering into a contract (Article 6(1)(a) and (b) GDPR).

Recipients of the personal data provided by you in the inquiry sent via the contact form may include entities to whom the Administrator has entrusted data processing (so-called processors), e.g., IT support services.

Personal data will not be transferred to a third country or international organization.

Personal data processed for the purpose of handling the inquiry will be stored for the duration of the correspondence justified by the type of inquiry (but no longer than 6 months from the date of completion of the correspondence), or until earlier withdrawal of consent for processing. Depending on the outcomes of the correspondence, the data will either continue to be processed for the purpose of contract execution—about which the data subject will be separately informed—or will be deleted if cooperation is not established.

The person whose data is being processed has the right to access their personal data and the right to rectify, delete, or limit processing, the right to data portability, the right to object to data processing, and if processing is based on consent: the right to withdraw consent at any time without affecting the legality of the processing carried out on the basis of consent before its withdrawal.

If the person whose data is being processed believes that the processing of their personal data violates data protection regulations, they have the right to lodge a complaint with the supervisory authority responsible for data protection, which is the President of the Personal Data Protection Office.

Providing personal data is voluntary.
